Requirements for Application
You can apply through Sun Shop in your are or Online via Sun website
1. Proof of Identification - at least 1 valid ID like SSS/GSIS/TIN ID, PRC ID, Driver's License, Philippine Passport and Company ID.
2. Proof of Billing - like Utility Bill (Electric, Water, Telephone and Cable/Internet), Mailed Bank Statements, Insurance Premium Statements of Account, Credit Card Statement, Notarized Contract of Lease, Airway Bill and Post-Marked Envelop. Must be of recent billing Statement with proof of payment if already due.
3. Proof of Income - Certificate of Employment and Compensation/ITR together with your two half month payslip (gross monthly income depends on the plan you will avail)
Note: Bring Original and Xerox for validation and all applications are subject to approval and additional documents may be requested.
